What you need to take to the Post Office or Passport Agency

  • DOWNLOAD and complete DS-11 Application Form
  • DOWNLOAD and SIGN 2 copies of RSVPpassports Letter of Authorization
  • Proof of U.S. Citizenship
    You will need one of the following documents:
    - A certified U.S. birth certificate
    - An expired U.S. passport
    - Original naturalization certificate
  • Proof of Identity
  • Passport Photos (2)
  • Proof of Itinerary
  • U.S. Government Fees
    You will need to write two separate checks or money orders.
    - Adult Passport: $170.00 payable to "U.S. Department of State" + $25.00 payable to the Post Office or Passport Agency
    - Adult Passport Card: $90.00 payable to "U.S. Department of State" + $25.00 payable to the Post Office or Passport Agency 
    - Adult Passport + Passport Card: $200.00 payable to "U.S. Department of State" + $25.00 payable to the Post Office or Passport Agency 

 

 

DO YOU NEED A NEW PASSPORT FOR A CHILD UNDER 16?
Below are special requirements for New Child Passport Application:

  • DOWNLOAD and complete DS-11 Application Form
    The minor and both parents (or guardians) must appear in person to submit Form DS-11 at the Post Office or Passport Agency. If one parent or guardian is not able to appear in person, one parent or guardian must submit the second parent or guardian’s notarized Statement of Consent (DS-3053). For more info, click here. 
  • DOWNLOAD and SIGN 2 copies of RSVPpassports Letter of Authorization
  • Proof of Child's U.S. Citizenship
    You will need one of the following documents:
    - A certified U.S. birth certificate
    - An expired U.S. passport
    - Consular report of birth abroad or Certification of Birth
    - Original naturalization certificate
  • Proof of Relationship between Child and Parent(s)/Guardian(s)
    You will need one of the following documents:
    -     Minor's certified U.S. birth certificate with both parents' names
    - Minor's certified Foreign Birth Certificate with both parents' names
    - Minor's Report of Birth Abroad with both parents' names 
    - Adoption Decree with adopting parents' names
    - Court Order establishing custody 
    - Court Order establishing guardianship
  • Proof of Identity of Parent(s)/Guardian(s)
  • Passport Photos (2)
  • Proof of Itinerary
  • U.S. Government Fees
    You will need to write two separate checks or money orders:
    - Minor Passport: $140.00 payable to "U.S. Department of State" + $25.00 payable to the Post Office or Passport Agency
    - Minor Passport Card: $75.00 payable to "U.S. Department of State" + $25.00 payable to the Post Office or Passport Agency 
    - Minor Passport + Passport Card: $155.00 payable to "U.S. Department of State" + $25.00 payable to the Post Office or Passport Agency
